If you're using GIMP some brushes allow you to rotate them, but you'll have to setup a shortcut for that. I tend to use Shift+[ or ] to rotate the brush left or right. Reason for this is GIMP currently to my knowledge does not have the ability to adjust the brush direction in accordance to direction or pen tilt.
