The best and easiest solution I have found, is to use an opensource program by the name of xNormal. Its free, super easy to use, and has great results. You literally import a high poly mesh, and a low poly mesh, tell it what maps you would like baked, and it will create them. This also works with textures, so you can keep your poly paint information and transfer it directly onto a UV sheet with no seams. The texture process is a bit more complicated, I believe you make a texture in Zbrush after you poly paint, load that, and then can bake it. If you are having trouble with it, let me know and I will try to figure out the workflow. I haven't used it in awhile, so I will have to do some experiments/ research.
