HELP my Tablet will only draw straight lines in photoshop cs4, has anyone else had this problem?

which wacom do you used?
i had the same prroblem before when i used my intuos 3,because the new driver doens't work in my pc/computer,finaly i try to installed the older version and everything work well. or try to update your driver from wacom with the new one.

the problem is they only have one driver for all.
i am pretty sure the problem is not in your tablet but the dirver(bug).

well normally that happens only when you have shift pressed down continuously, also when your pressure or transfer is inactive (so the pen won't acknowledge any difference in pressure).
In those strange cases I would suggest, trying to re-start the computer, if that doesn't help reinstall photoshop and make sure you have the latest drivers for your tablet, and if they don't work try with previous version of the drivers.
Also every time you install a driver, remember you have to re-start the computer so the new driver takes effect.
